Truly a critical review: root cause analysis of paraplegia following transforaminal epidural steroid injections: "unsafe" triangle.

چکیده
We read with great interest the article by Glaser and Shah titled “Root Cause Analysis of Paraplegia Following Transforaminal Epidural Steroid Injections: The Unsafe Triangle (Pain Physician 2010; 13:237-244). For years, the well known “safe triangle” (when performing transforaminal lumbar epidural steroid injection) has been taught and learned by hundreds of interventionists in International Spinal Interventional Society (ISIS) training courses, teaching seminars, books, journals, educational videos, as well as in pain fellowship programs. This technique is considered “bread and butter” of interventional pain management. As fellowship trained, board certified interventional pain specialists, we were more than stunned by what was presented in this perspective review. Root cause analysis of paraplegia following transforaminal epidural steroid injections: the 'unsafe' triangle.
The utilization rate of transforaminal epidural steroid injections (TFESIs), an elective diagnostic and therapeutic spinal procedure, has risen dramatically over the past decade. In 2006 alone, greater than 300,000 thoracolumbar TFESIs were performed on Medicare beneficiaries. Despite the purported superiority of the transforaminal route, compared to other modes of epidural injection, TFESIs ar...
متن کاملParaplegia following a thoracolumbar transforaminal epidural steroid injection.
BACKGROUND In recent years, transforaminal epidural injections have emerged as an alternative to interlaminar and caudal epidural steroid injections. The rationale for utilizing transforaminal epidural injections has been described for diagnostic as well as therapeutic purposes. The evidence for lumbar transforaminal epidural steroid injections in managing lumbar nerve root pain is strong, wher...
متن کاملDescription of Optimal Angle of Needle Insertion For L5 Transforaminal Epidural Injection Leads to Complications.
Ra and Min (1) painstakingly have described the optimal angle of needle insertion for fluoroscopically guided transforaminal epidural injection of L5. Unfortunately, this potentially increases further complications related to lumbar transforaminal epidural injections with the safe triangle approach (2).Clearly, the literature undeniably shows that the safe triangle approach may not be safe and ...
متن کاملA critical appraisal of the evidence for selective nerve root injection in the treatment of lumbosacral radiculopathy.
OBJECTIVE To critically review the best available trials of the utility of transforaminal epidural steroid injections (TFESIs) or selective nerve root blocks (SNRBs) to treat lumbosacral radiculopathy. DATA SOURCES MEDLINE (PubMed, Ovid, MDConsult), EMBASE, and the Cochrane database. Databases were searched from inception through 2003. STUDY SELECTION A database search was conducted by usin...
متن کاملAlternative approach for lumbar transforaminal epidural steroid injections.
The traditional superoanterior approach for transforaminal epidural steroid injection (TFESI) carries a risk of complication by obstructing arterial flow to the anterior portion of the spinal cord by puncturing the spinal radicular artery that passes through the superoanterior foraminal zone or "safe triangle" zone, which does not describe vascular safety, but rather describes neural safety. Co...
